The Results of Exchange Surgery in the Eyes with Opacified Intraoculerintraocular Lens

3Vakıf Gureba Eğit. ve Araş. Hast., Göz Has. Klin., İstanbul, Asist. Dr. Purpose: To evaluate the clinical outcome and complications of exchange surgery in eyes with opacified hydrophilic acrylic intraocular lenses (IOLs).
Materials and Methods: Twenty - eight eyes of 22 patients which that had been implanted with the same brand of IOL (Intraocular Optical International, California) underwent a exchange surgery because of significant deterioration of vision related to the calcific opacification. Intraoperative and postoperative complications were evaluated.
Results: In the 17 eyes opacified IOLs were explanted totally, and in 9 acrylic foldable IOLs and in 8 polymethylmethacrylate IOLs were implanted.. The opaque acrylic IOLs could only be removed with the entire capsule in 3 eyes. In the remaining 8 eyes IOLs were explanted by cutting the optic from the haptics followed by implantation of hydrophobic acrylic IOLs in the ciliary sulcus. Intraoperative complications were zonular dialysis in 4 eyes and posterior capsular tears in 4 eyes. Mean preoperative and postoperative best corrected visual acuity (BCVA) (decimal) values were 0.0963 ± 0.0689 SD (range 0.016-0.3) and 0.64 ± 0.26 SD (range 0.2-1.0), respectively. The visual improvement was statistically significant (Wilcoxon sign rank test p<0.0001).
Conclusions: IOL exchange surgery for opacified IOLs can yield a significant improvement in the visual function of patients. However, the ophthalmic surgeon should carefully analyze the concordance between the density of IOL opacification, the preoperative visual acuity, and co-morbidity of the eye. Moreover, the complaints and vision needs of the patient should also be considered. Keywords : Opacified acrylic lens, lens exchange
